Connecticut and Rhode Island Residents
Connecticut residents who are blind, disabled, qualifying veterans, or guardian to a disabled person, and non-residents under qualifying conditions, can request an accessibility parking permit online. You can request a renewal of an accessibility parking permit up to 6 months prior to its expiration and within 60 days after its expiration. Temporary permits cannot be renewed. The same is true for Rhode Island residents.
In addition to requiring a Connecticut drivers license or non-driver id number, a valid email and mobile phone number is needed. The same is true for Rhode Island applicants.
Handicap Placard service pricing of $250 includes medical records review for eligibility and state form completion for submission by you to state. If you wish Modern Medical LLC to submit the completed application on your behalf, there is an additional $50 fee. All fees are payable in advance prior to the medical records review and form completion.
Individuals interested in obtaining a handicap placard must first have a virtual consultation with our medical professional. Should you wish to proceed, you will need to provide medical records indicating that you meet at least one of the 6 criteria shown below. Our provider will review the medical records and if in agreement will complete the state Handicap Placard application form. This form will be forwarded to you or can be submitted on your behalf to the state for an additional filing fee.
State Criteria for issuing a temporary or renewable handicap placard are:
A. The applicant is blind (Must be certified by an optometrist, ophthalmologist or by Board of Education and Services for the Blind- BESB); OR
B. The applicant has a disability that limits or impairs their ability to walk. These conditions are defined in 23 CFR 1235.2 and are listed below :
1. The applicant cannot walk two hundred feet without stopping to rest; or
2. The applicant cannot walk without the use of, or assistance from, a brace, cane, crutch, another person, prosthetic device, wheelchair, or other device; or
3. The applicant is restricted by lung disease to such an extent that the person's forced (respiratory) expiratory volume for one second, when measured by spirometry, is less than one liter, or the arterial oxygen tension is less than sixty mm/hg on room air at rest; or
4. The applicant uses portable oxygen; or
5. The applicant has a cardiac condition to the extent that the their functional limitations are classified in severity as Class III or Class IV according to
standards set by the American Heart Association; or
6. The applicant is severely limited in the ability to walk due to an arthritic, neurological, or orthopedic condition; OR
C. The applicant is a veteran with PTSD and a disability that limits or impairs the ability to walk, as defined in # B1 through 6 above (PTSD and veteran status must be certified by a psychiatrist with the U.S. Department of Veterans Affairs-USDVA)

Due to the increased regulatory burdens, our providers will NOT prescribe chronic pain medications.
If your automatic credit card charge fails, there will be a $25 processing fee each time there is a failed payment. Two consecutive failed charges will result in a discharge from the practice.
It is essential that you and your family maintain a medical insurance policy to cover such items as hospitalizations, services provided by specialists, diagnostic testing and prescription drugs.
