Handicap Placards, Employer and School Physicals, Other Forms
Our medical form completion services make healthcare simple, convenient, and stress-free. We handle a wide range of forms, including handicap placards, school and camp physicals, pre-employment and CDL physicals, and other required medical documentation.
Our initial consultation regarding our medical form services may be conducted via virtual appointments. Medication and Prescriptions are NOT included.
Sports and camp physicals helps ensure that your child is healthy and physically prepared to participate. The goal is to help reduce the risks of potential injury and these types of physical exam are not intended to replace a comprehensive annual wellness exam and other screenings.
Payment is expected prior to the release of the form to the patient. As each form is unique as to what is required, please contact us directly in order that we may provide you with the most accurate pricing and timing for the form completion.
The sports and camp physical exam is not intended to replace a comprehensive annual wellness exam and other screenings.
Modern Medical's nurse practitioner will review your child's medical history, conduct an exam and recommend additional tests if needed. Following the session, your provider will complete the required medical forms for you to give to the organization.
Camp and Sports Physicals with completed medical form are $90 per child. Group discounts are available for team organizations.
The Department of Transportation (DOT) physical exam at Modern Medical determines if you’re medically qualified to operate a commercial motor vehicle, per DOT regulations. The DOT physical exam is not intended to replace a comprehensive annual wellness exam.
The nurse practitioner will check your blood pressure and pulse and listen for an irregular heart rate. Your hearing must allow you to hear a 'forced whisper' at 5 feet (or have less than 40 decibels hearing loss in the better ear). A urine test will screen for underlying medical conditions such as diabetes. Finally, your eyesight will be tested; you must have a minimum acuity of 20/40 in each eye and at least 70º of peripheral vision in each eye, with or without corrective lenses.
The CDL Physical is $150 initial visit and $50 for return visit where you will receive your completed medical form .

Late Cancelation/No Show Fee is $99 per person as we have reserved this time for you.
Due to the increased regulatory burdens, our providers will NOT prescribe chronic pain medications.
If your automatic credit card charge fails, there will be a $25 processing fee each time there is a failed payment. Two consecutive failed charges will result in a discharge from the practice.
It is essential that you and your family maintain a medical insurance policy to cover such items as hospitalizations, services provided by specialists, diagnostic testing and prescription drugs.
